How to fill in and find your passport form on passport.moi.gov.af
To get an Afghan passport inside Afghanistan, you first fill in a form on the passport department's website, passport.moi.gov.af. At the end you can download your filled-in form and a bank slip. This guide shows what to have ready, how to fill each part, how to find a form you already sent, and what the site does and does not tell you.

What to have ready
The department's own guidance (checked 2026-10-09) lists these for the application:
- Your tazkira (paper or electronic) or a birth card. A paper tazkira must be confirmed by the civil registration office of the National Statistics and Information Authority; an electronic tazkira does not need that.
- A photo: colour, 4 × 5 cm, white background, taken straight on with both ears visible, no glasses or lenses, no filters or editing, and not older than 6 months. The online upload must be under 240 KB.
- The details exactly as on your tazkira: first name, surname, exact date of birth, place of birth, tazkira number, phone number and height.
- For a child under 18: a written request from a parent, guardian or legal representative. A mother must prove she is the mother with the birth card or the marriage certificate. A child of 7 or older needs a tazkira with a photo.
The department says that if a passport is printed from wrong details, or you ask for a change that needs a new passport, you pay the fee again. Check every letter before you submit.
Fill in the form
The form is at passport.moi.gov.af. The department's registration guide takes you through these steps:
- Photo. Choose your photo, adjust it in the frame and save it.
- Name. Write your name in both Dari and English boxes. Each box turns green when it is filled in correctly.
- Date of birth. Pick it in the Solar Hijri calendar or the Gregorian one. The site converts the other for you.
- Place of birth. Choose the country, then the city or province.
- Identity document. Choose the type, enter its number and choose your home area. For an electronic tazkira the site adds the dashes itself.
- Personal details. Sex, height, job, hair colour, eye colour and marital status.
- Type and length. The site shows the passport types and periods open to you.
- Where you will go. Choose the province where your papers will be processed and the passport collected. In some provinces you can choose home delivery or a post office. Home delivery costs 100 afghanis more.
- Address. Write the address for the passport. The form shows what you must pay.
- Check and submit. Press the button that shows your details, read them, enter the code asked for and confirm. Then download the form if the button is active.
Find a form you already sent
Open the form search page. It is for people who have already registered. You can search two ways:
- By code: your form code and your date of birth (Solar Hijri or Gregorian).
- By details: your name, your father's name, your grandfather's name and your date of birth.
The site's pages do not say what the result shows or whether it gives a status such as "approved". Do not trust any list of "statuses" from another site. If you cannot find your form, ring the passport department's headquarters on 020-2500400 or 020-2510984.
Pay and collect
After you register, print or keep a copy of the form and take it to an agency of the banks the department names, in Kabul or in your province, and pay the set amount against it. The department lists these amounts (2026):

- Ordinary passport, 10 years: 10,000 afghanis
- Post service: 200 afghanis in Kabul, 500 in the provinces
- Red Crescent contribution: 200 afghanis
- Home delivery, if you choose it: 100 afghanis
Children pay a reduced fee, and the form shows the amount for you. For the exact total, use the passport tool. You must also go in person to the passport headquarters or the provincial office for your fingerprints and to show your documents. The passport is then sent to the post office you chose: in 2024 the Christian Science Monitor reported that the postal service hands out up to 15,000 passports a day.

Who cannot get a passport
The department says no passport is issued to someone who owes the government more than 50,000 afghanis, who is under a court travel ban, or to a child with no parent, guardian or legal representative. If the debt is cleared or the ban lifted, the office concerned must write officially to take you off the list.
Do not pay anyone else
The department says to pay only the amounts above. If anyone asks for more, report it to 020-2510984 or 020-2500400, or to the police on 119. Sites and agents that offer to "check", "speed up" or "book" your form for money are not the passport department. The official address is passport.moi.gov.af, and nothing else.
The department began asking applicants to apply online in December 2021, to reduce crowding at its offices (Khaleej Times).
